Are you talking about the golden leaves pin? if you are its referring to him being a Major in the Marines.

Mac's pen has a D on it for "Detective". I've asked the same question before and was told it is given to Detectives who make first grade. Most detectives are third grade, second is the equivelant of a Sargeant and first is the equivelant of a lieutenant. The man who answered my question was a cop, so I'm pretty sure he was accurate.
